The "Return of Dr. Octagon" album isn't a real Kool Keith album. Basically his label took some of his vocals, put them over third-rate beats, and then let a bunch of unknowns remix the tracks, all without his or his camp's approval.
That Mr. Nogatco album that came out last year is the more fitting sequel.
